Eating Disorders: Types, Warning Signs, and Why Early Help Matters

Eating disorders are among the most serious mental health conditions that exist — and among the most misunderstood. They are frequently reduced to caricature: a teenage girl restricting her food intake to look like the images in a magazine. The reality is far more complex, affects a much broader population, and carries significant medical and psychological severity. Eating disorders have the highest mortality rate of any psychiatric condition, and early identification and treatment meaningfully improve outcomes.

What Eating Disorders Are — and Are Not

Eating disorders are not lifestyle choices or diets gone too far. They are serious psychiatric conditions characterized by severe disturbances in eating behavior and related thoughts and emotions, typically involving an intense preoccupation with food, weight, and body image.

They are not exclusively conditions of young women. Eating disorders affect people of all genders, all ages (from children through older adults), all races and ethnicities, and all body sizes. The cultural image of eating disorders as thin, white, adolescent-female conditions has contributed to systematic underdiagnosis in men, people of color, older adults, and people in larger bodies — all of whom can have equally serious eating disorders that are missed or dismissed.

They are also not vain or superficial concerns. The thoughts, rituals, and behaviors associated with eating disorders are not chosen and are not simply correctable with better self-esteem messaging. They involve significant psychological distress and often have neurobiological underpinnings that make recovery genuinely difficult without specialized treatment.

Common Types of Eating Disorders

Anorexia Nervosa is characterized by restriction of energy intake leading to significantly low body weight, intense fear of gaining weight or becoming fat (even when already underweight), and a distorted experience of one's own body weight or shape. Anorexia has the highest mortality rate of any psychiatric condition, with deaths occurring from medical complications of starvation as well as suicide.

Crucially, a person does not have to appear visibly underweight to have anorexia or a similarly serious restrictive eating disorder. People in larger bodies can have atypical anorexia (now classified in the DSM-5 as "other specified feeding or eating disorder") with all of the same restrictive behaviors and psychological features and equally serious medical consequences, and they are frequently not identified because their appearance does not match the stereotype.

Bulimia Nervosa involves recurrent episodes of binge eating (eating a large amount of food in a discrete period with a sense of loss of control) followed by compensatory behaviors to prevent weight gain — most commonly purging through self-induced vomiting, but also laxative or diuretic misuse, fasting, or excessive exercise. People with bulimia are often at a normal or higher body weight, which contributes to the condition going unrecognized. Medical complications of bulimia include electrolyte imbalances that can cause cardiac arrhythmia, esophageal damage, and dental erosion.

Binge Eating Disorder (BED) is the most prevalent eating disorder and involves recurrent binge eating episodes without the compensatory behaviors characteristic of bulimia. BED involves significant distress about the binge eating and is associated with substantial physical and psychological consequences. It is not simply overeating — the experience of loss of control and the associated shame and distress are defining features.

Avoidant/Restrictive Food Intake Disorder (ARFID) involves restriction of food intake not driven by body image concerns but by a lack of interest in eating, sensory sensitivity to certain food characteristics, or fear of aversive consequences of eating (choking, vomiting, or allergic reactions). It occurs across the lifespan and is distinct from picky eating by its severity and impact on functioning.

Other Specified Feeding or Eating Disorders (OSFED) is a category that captures clinically significant eating disorder presentations that do not fully meet criteria for the above diagnoses. This category includes many people with serious conditions who have historically been undertreated because they did not fit a specific diagnostic mold.

Warning Signs

Because eating disorders are often concealed with considerable skill, identifying them in someone you care about requires attention to patterns rather than obvious behaviors:

• Significant changes in eating behavior: skipping meals, adopting severely restrictive diets, avoiding eating with others, rigid food rules
• Excessive concern with food, calories, weight, or body shape
• Disappearing to the bathroom after meals
• Wearing loose or layered clothing to conceal the body
• Excessive exercise, particularly exercising despite injury or illness, or intense distress when unable to exercise
• Physical signs: feeling cold constantly, loss of menstrual cycle, hair loss, lanugo (fine hair on body), calluses on knuckles (from purging), swollen jaw
• Marked change in mood, social withdrawal, increased secrecy
• Obsessive research about food, nutrition, or diet
• Denial of hunger or problems with eating

Getting Help

Eating disorders require specialized treatment — not general counseling or individual therapy from a practitioner without specific training in eating disorders. Treatment typically involves a multidisciplinary team including a therapist who specializes in eating disorders, a registered dietitian, and a physician who can monitor medical status.

Evidence-based treatments for eating disorders include Family-Based Treatment (FBT) for adolescents with anorexia, Cognitive Behavioral Therapy for Eating Disorders (CBT-E), and Dialectical Behavior Therapy (DBT) for presentations involving emotion dysregulation. Higher levels of care — intensive outpatient programs, partial hospitalization, or residential treatment — are often required for medical stabilization or when outpatient treatment has not been sufficient.
